How do I find information on the website?

A. The main menu is located at the top of the screen. For each section there may be additional information available. You can find this information by looking in the bright green box which is located on the lower right side of the menu.

Why can't I view the questionnaire?

A. The questionnaire has been tested across several browsers. There are no known issues when using Mozilla 1.3+ (all operating systems), Internet Explorer 5.0+ (Windows 95/98/ME/NT/2000/XP/MAC), Netscape 7.0+ (all operating systems), Opera 7.0+ (all operating systems), and Safari 1.0+ (Mac OS X 10.x).

However, there are known issues when using the AOL Internet Browser. You may not be able to view the questionnaire. There is no known resolution for this problem at this time. If you currently use the AOL browser it is suggested that you fill out a paper copy of the survey instead. Please contact Valerie Monet for further information at 313-577-5858.

When I logged in the information was not for my organization. What do I do?

A.Log out and log in again. If the same thing happens please contact Dale Thomson at 313-577-0298 with your username, password, and the name of your organization.

How is my information saved in the system?

A. Each time you click the button .save and proceed. the information you enter on the page is saved to our database. If you are in the middle of completing your survey and you decide you would like to complete it later, you must click save and proceed to save the data that you entered on the page.

I entered information on a page, but when I saved I was kicked out of the system. Why did this happen?

A. The system will time out if you leave it on one page for a long period of time without activity. It will automatically log you out without saving data you entered on the page. You should gather all information you will need to fill out the questionnaire prior to working with the online system. This will resolve any problems you have with the system timing out, as no page should take longer than a few minutes to fill out.

Also, if you are using a dial-up connection or there is a disturbance in your high-speed internet connection and your computer goes offline briefly you will be logged out of the survey.

Can I save my questionnaire and come back later?

A. Yes, but before you log out of the survey please be sure to click "save and proceed". This is the only way that the data you entered will be saved.

I have completed the questionnaire, how should I submit it so that you know I am done?

A. On the last page of the survey there are two options when logging out of the system. First, you will see the "Form is Complete" button. If you click this evaluation staff will be notified that you have completed the questionnaire and are submitting it.

The second button, "Complete Later" does not notify staff of your progress. Only log out using this option if you plan to log in later and modify your survey.

Why can't I view the CPC Reports?

A. CPC Reports require a username and password (not your username and password to complete the survey) to be viewed. Only authorized individuals may access these reports. If you are authorized to view these reports and you are experiencing technical difficulties please contact Dale Thomson at 313-577-0298.
